Forklift Safety Inspection Checklist



When to Complete the Checklist

A checklist must be completed on each forklift at the beginning of each shift. Even if the forklift has been in service on the previous shift, a new checklist must be completed. All checklists should be reviewed by either a member of management or the maintenance department or both. These checklists should be kept on file for a minimum of five years. Should you be audited by OSHA, they may wish to see these checklists to ensure your program is being managed properly.

Items On the Checklist

Each facility can develop its own checklist to include any number of items. Some employers will require more extensive checks be completed, while others will require only the minimum checks be made. At the very least, these checklists should include checks of the tires, horn, lights, hydraulic hoses, alarms and warning lights as well as the smooth operation of all controls. Once these items are checked with the machine off, additional items will need to be checked with the machine running. These will include the brakes, steering, smooth movement of all hydraulic equipment and the emergency brake. When the machine is first moved the operator should check the floor for any signs of leaks.

Post-Checklist

Once the checklist has been completed it should be turned into the proper person or office. Any problems detected must be noted on the checklist. If a problem is found repeatedly, it must be written on the checklist each time it is found. If a major problem is found, the operator should contact a manager. The forklift should not be operated until the problem has been evaluated by a qualified person and all necessary repairs or modifications made.
